By signing below, you either agree that you have obtained a physical examination and are in good health and able to participate in all classes, activities or events or you have agreed to forgo such an examination and represent to BCKM that you are in good health and able to participate in all classes, activities or events.
You further represent that you understand the inherent danger in participating in any class, activity or event, including severe bodily injury or death, and voluntarily assume all such danger arising out of or relating to in any way such activities.
You further agree to forever waive, release and discharge any and all claims or causes of action of any kind that you may have against Boise Cities Krav Maga Inc., staff, instructors, employees, agents and volunteers, arising out of or relating to in any way such activities. Finally, by signing below you acknowledge that you have read the foregoing, understand it and agree to bound by these terms.

I understand that both still photography and videography are used in our marketing. I understand that myself or my child may be featured in a photo or video on social media. I understand that there is no compensation for being used in a photo or video. If I am featured in a photo or video, and do not want to be, I can request for it to be removed immediately.
